Use this form for original applications or renewals of parking permits.
Renewals are every four years for permanent and every six months for temporary. The form may also be used to obtain the disabled license plate.
Once the form is completed, submit it to our office within 12 months along with a copy of your valid driver license or identification card for processing by mail to:
